We accept Medicaid, Medicare, Healthy Kids/Florida KidCare, and most private insurance plans. Any remaining balance will be the responsibility of the patient.
Sliding Fee Discount Program
Premier offers a Sliding Fee Discount Program for eligible patients who are uninsured or underinsured. Discounts are based on income and household size. Proof of income must be presented to qualify.

Our nominal fees will be slightly updated to help us maintain the quality of care you deserve.
Examples at the deepest discount level:
- Primary Care & Medical Visits: $45 (previously $25)
- Behavioral Health Visits: $30 (previously $20)
